Conditions for obtaining service

  • The applicant must be a holder of a Professional License as an optician.
  • If the applicant is not fully time dedicated to work in the shop/center, or if the applicant is not professionally licensed as per the intended scope of work in the shop/center, a professionally licensed technician must be appointed as an optician.  
  • The shop/center area must be no less than (12) square meters.

Procedures

  1. The applicant visits Health Institutions Licensing Department, fills out the form of licensing an optical center, and submits the letter of intent from the Ministry of Industry and Trade.
  2. The applicant documents the letter in the register of Health Professions and Institutions Licensing Directorate. The letter will then be forwarded to the Director of Health Professions and Institutions Licensing Directorate for review and then to Health Institutions Licensing Department.
  3. Once the letter is forwarded to Health Institutions Licensing Department, a decision is made.
  4. If the letter is approved, the applicant will receive an approval letter to register the intent.
  5. The applicant shall visit Health Institutions Licensing Department and submit the commercial trade name and register.
  6. The Optical Centers Licensing Committee studies the application, schedules a date of inspection and prepares a report to state so.
  7. Check if the fittings of the center/shop comply with the requirements of licensing optical centers no. (6) for the year 1986.
  8. The applicant receives the approval letter signed by the Minister of Health for licensing and direct action.

Required Documents

  • The letter of intent from the Ministry of Industry and Trade.
  • A copy of the Professional License for the responsible technician or technicians.
  • A letter of appointment of the responsible technician or technicians.
  • A letter of pledge from the technician or technicians to take responsibility for the work in the shop/center.
  • A copy of ownership deed or lease contract.
  • A survey plan of the shop/center.
  • A copy of the commercial register and the trade name registration certificate.
